Almost three months after it closed down its OPD services, All India Institute of Medical Sciences (AIIMS) has decided to resume the operations from June 25 for its follow-up patients. Initially, each department will see not more than 15 patients a day, officials said on Tuesday.

The outpatient department (OPD) services will be available for follow-up patients as of now, but requisite appointments for limited number of new patients will also be given for those departments which wish to start physical OPD consultation, they said.

For the first time in its history, AIIMS had shut down its OPD services, including specialty services, as well as new and followup registrations from March 24 as part of its effort towards redirecting its resources to control the Covid-19 outbreak.

“AIIMS to restart physical OPD services from June 25. It is informed that the Director AIIMS has approved the resumption of physical appointments for old/follow-up patients, not to exceed 15 per day for any department initially which will be enhanced in a few days,” an official circular issued to all departments on Tuesday said.

In addition, requisite appointments for limited number of new patients will also be given for those departments which wish to start physical OPD consultation, the circular stated. However, no appointments will be given for evening specialty clinics in the first phase of OPD reopening.

“It will be the prerogative of the departments to call patients directly or screen them through tele-consultation before giving a physical appointment,” the circular said.

Patients can be given appointment for OPD consultation directly by the department or through the computer facility as decided by the department concerned. However, if an appointment is given from the department, the departmental appointment list will have to be intimated to the computer facility and faculty in-charge of the OPD 48 hours in advance, it said.

All heads of clinical departments in AIIMS have been requested to provide information on the date of resumption of physical OPD for patients of the department concerned and daily list of patients along with their telephone numbers who have been given appointment for physical consultation, the circular added.

Doctors at AIIMS are currently providing tele-consultancy services to all patients undergoing treatment at the hospital in an effort to reduce the need for them to visit the facility during the lockdown. In addition, emergency procedures are being conducted on a regular basis.

Following the state government guidelines, those arriving in Gurgaon from other states for a stay of more than three days will now have to register on the Saral Haryana portal.

However, the rule doesn’t apply to those entering Gurgaon every day for work and business, as long as they don’t stay back in the city.

On June 17, the Manohar Lal Khattar government had decided to regulate inter-state movement of people entering Haryana for a stay of more than three days. On the portal, visitors will have to give their details, including name, address of origin and destination, and mobile number. They will also need to provide details of the accommodation type where they intend to stay during their visit. This applies to those who come to stay in the city with their friends, family members or relatives.

Visitors will have to share information about their health condition. Private and government guesthouses, hotels and dharamsalas in the city have been directed to register visitors from other states who intend to stay for more than three days on the Saral Haryana portal. Delhi government has started giving pulse oximeters to Covid-19 patients with mild and no symptoms who have been advised home quarantine by doctors. District officials have already distributed 6,000-7,000 pieces, a senior government official said.

Pulse Oximeter is a clip-on device that is attached to the fingertip to measure the oxygen level in the body of a patient infected by novel coronavirus. The oxygen concentration in blood is supposed to be between 95% and 100%. If it falls to 90% or below, the person requires immediate hospitalisation.

The coronavirus attacks the respiratory system, and if the infection becomes severe, a patient starts feeling breathless. Hence, doctors advise that patients should keep measuring the oxygen level every one-two hours and immediately contact district health teams if it falls below the acceptable level.

According to a senior official, Delhi government has ordered 75,000 pulse oximeters and around 46,000 have already come. “The devices are being distributed through chief district medical officers (CDMOs). A family with more than one patient will get one device,” said an official. The CDMOs have been given 29,000 oximeters and the rest will be available by the end of the week, said the official.

Once the patient recuperates, the device will have to be returned through Asha workers for sanitisation and further distribution. Each pulse oximeter is costing the government Rs 1,300, said an official.

Senior IAS officer Sanjay Goel, who is the nodal officer for procurement and supplies, said the government had also bought oxygen concentrators and ventilators to upgrade health facilities in the capital.

Goel said 4,600 oxygen concentrators were being procured, out of which 2,000 had already been received and distributed among district officials. If a patient in home isolation notices a fall in oxygen level, he will call up a centralised number and the district authorities will send a medical team with the device.

A portable oxygen concentrator is used to provide oxygen therapy to the people who require greater oxygen concentrations than the levels of ambient air. “Once the therapy is given, it will be returned to the district authorities. It is a reusable device and will be sanitised thoroughly before sending it to another patient,” Goel said.

However, the patient will immediately be hospitalised if the oxygen level does not come back to normal despite the use of the concentrator.

How can the Tamil Nadu government expect private schools and colleges to pay salaries to their staff if the institutes are prohibited from collecting even minimum fees, the Madras high court asked on Tuesday.

“It is the policy decision of the government that all private educational institutions must pay salaries to their staff, both teaching and nonteaching. But if unaided institutions are prohibited from collecting even minimum fees, how will they pay salary, that too when almost all such institutions are conducting online classes,” Justice R Mahadevan asked.

“When teachers are pressured to conduct online classes, shouldn’t they be paid,” the judge asked, while admitting the plea moved by the Federation of Association of Private Schools in Tamil Nadu and All India Private Educational Institutions Association and Consortium of Self Financing Professions Arts and Science Colleges in Tamil Nadu.

The court then directed the state to respond to the pleas by June 30.

Earlier, representing the petitioners, senior counsel K M Vijayan submitted that on April 20, the state issued a government order restraining all private schools and colleges in TN from demanding fee, payable by students as fixed by the fee fixation committee, citing the Covid-19 outbreak.

At the same time, the state had also instructed all such schools to pay salaries to all their staff without any default, Vijayan said.

“Wherever there is a right, there is an obligation. Similarly, wherever there is an obligation, there is a right,” he said and asked how these institutions, whose only source of income is fees from students, can pay salaries when they are prohibited from collecting the same.

Vijayan further added that the Disaster Management Act, 2005, under which the GO has been issued was solely enacted for managing disaster and it had relevance towards the financial management of individuals or schools/colleges management.

Only in Tamil Nadu was such a prohibition imposed. All other states have permitted such institutions to collect fees, he said.

Delhi: To keep in check whether the Medical Colleges have the minimum standard requirements which are necessary for the yearly admission process, the Medical Council of India (MCI) is reported to have decided to constitute a committee of experts. As per recent media reports, this committee's objective is to review Minimum Standard Requirements Regulations and phase-wise requirements of the Medical Colleges for 50/100/150/200/250 admissions annually. 

Hitavada reports that according to the MCI order, the committee comprises members of medical faculties, medical experts and officials who have experience in the field of medical training and it will affirm that the concerned institutions are following the norms and regulations which ensures proper medical training. The committee will be responsible for maintaining the rules and regulations of the Indian Medical Council Act and will introduce modifications accordingly. 

The Indian Medical Council Act was introduced in 1956 for the reconstitution of the Medical Council of India, and the maintenance of a Medical Register for India and for matters connected therewith; however, with time as circumstances changed, many aspects which were made mandatory by the act like- the number of beds, qualification of faculty, the size of lands has also changed. Hence, the government decided to form this new committee which will consider the current situation while fixing norms and regulations. Moreover, the committee will also monitor resources for training, hardware required for training, technological advancement to help current training and preparing techniques, the clinical material required for preparing the medical students. The committee may also imbibe changes in the administrative structure 

While Dr Suresh Chandra Sharma, Chairman-designate, National Medical Commission is the chairperson of the committee, Dr S Ramji, Ex-Dean, MAMC and presently Senior Advisor, BoG, MCI, will act as Co-Chairperson, reports The Hitavada. Atal Dulloo (Financial Commissioner, H&ME, J&K), Dr Anna B Pulimood (Principal, Christian Medical College, Vellore, Tamil Nadu), Dr Sharad Kumar Rao (Dean, Kasturba Medical College, Manipal, Karnataka), Dr L Fimate (Retired Director, Zoram Medical College, Aizwal, Mizoram and Professor of Forensic Medicine, Imphal, Manipur), Dr Achal Gulati (Ex-Principal, BSA Medical College, New Delhi and Professor of ENT, MAMC, New Delhi), Prof Deelip G Mhaisekar, Vice-Chancellor, Maharashtra University of Health Sciences, Nashik, Maharashtra), Dr Ranjeet Sinha (Associate Professor, Department of Community, Patna Medical College, Patna, Bihar), Dr Rajneesh Dubey (Principal Secretary, Medical Education, UP Government), SS Shukla (Principal Secretary, MP Government) and Devesh Deval (Ex-Director, Medical Education, Union Ministry of Health and Family Welfare) are members of the committee, adds Daily Excelsior According to the order, the committee may consider amalgamation and simplifying the existing regulatory framework including phase-wise requirements and recommend revision of the same in view of the current health scenario of the country. Besides, the committee may consider any other incidental matter too.
